Initial Examination

Please bring a letter of introduction and your health insurance card for your first visit.

If you do not have a letter of referral, you will be asked to pay a special fee of 5,500 yen (including tax) as an uninsured combined medical treatment fee for the initial visit, in addition to the medical fee.
For details, please refer to "Selective Medical Treatment Fee".

Please be sure to bring your insurance card and medication book with you when you visit the hospital.

1 Fill out the application form for the first medical examination and check your insurance card.

Please fill out an application form for an initial medical examination and present your insurance card at the general reception desk on the 1st floor.
If you have a letter of referral, please come to the referral desk.

2Receipt for the department and filling out the medical questionnaire

Please present your receipt to the counter of the department you wish to see.
Please fill out the medical questionnaire.

3Examination

Please enter the examination room when the number on the reception slip is displayed on the waiting display board in front of the examination room.
After the examination, please wait in the waiting area until you are informed by the office staff.

4Accounting

When your bill has been prepared, the number of your receipt will be displayed on the billing machine, and you can pay at the automated teller machine on the 1st floor or at the checkout counter on the 1st floor.

Re-examination

When a patient's condition has stabilized, we will refer him or her to another medical institution. However, if the patient continues to visit our hospital on his or her own judgment, a special fee of 2,750 yen (including tax) will be charged in addition to the medical fee for the second visit as a combined medical treatment fee not covered by insurance.

Please use the Reception Machine on the 1st floor to obtain a Reception Slip for the department you wish to visit.
Please keep the receipt until the time of payment.

1Referral Reception

Please come to the "Referral Reception" on the 1st floor.
You will be given a number slip, so please ask the Referral Receptionist first.
*If you have to cancel or change your appointment, please contact us through your referring medical institution.

2Department Reception

Please hand the purple file you received at the "Referral Reception" to the counter of the department. You will receive a receipt at the counter of the department.

*The number on the receipt will be displayed on the waiting display board and the accounting number indicator, so please wait for the receipt until the time of payment.

3Consultation

Please enter the examination room when the number on the receipt is displayed on the waiting display board before the examination.
After the examination, please wait in the waiting area until the office staff informs you of your number.
After receiving your prescription, please go to the 1st floor accounting area.

*The medical information form will be either handed to you on the same day or mailed to the referring medical institution at a later date.

4Accounting

When your account has been prepared, the number of the reception machine will be displayed on the account number display machine on the 1st floor. When the number is displayed, please pay the bill at the automated teller machine or at the accounting counter.
